Position Title: Case Manager
Part-time from 9:00 am to 3:00 pm; Monday – Thursday with flexibility

Our Case Managers play a critical role in connecting clients with resources and services, and ensuring every individual receives the necessary support to address their needs.
Position Summary
The Case Manager is responsible for providing direct client services, including intake, assessment, and referrals to other community partners. This role requires strong communication skills, attention to detail, and the ability to work effectively with diverse populations. The Case Manager ensures accurate documentation, maintains compliance with program requirements, and collaborates with community partners to support client success.This position requires the Case Manager to work onsite at the appointed office location in Brownsville; this is not a work from home position.
Key Responsibilities
- Conduct client intakes, needs assessments, and eligibility screenings
- Provide referrals to community resources, including housing, food assistance, employment, transportation, and healthcare
- Maintain accurate, timely case notes and documentation in accordance with agency and funder requirements
- Prepare reports, and ensure compliance with grant or program guidelines
- Participate in meetings, training, and community outreach activities
- Meet and work with community partners
Qualifications
- Associate or bachelor’s degree preferred and/or related experience.
- Experience working with vulnerable populations preferred
- Strong communication, problem-solving, and organizational skills
- Ability to maintain confidentiality and handle sensitive information
- Proficient with computers/Microsoft 365, case management, and documentation
- Ability to work independently and as part of a team
- Valid TN driver’s license and reliable transportation
